Medical Insurance
We offer a choice of three pre-tax Presbyterian Healthcare Plans to qualified employees scheduled to work at least 20 hours per week. Dependent coverage also is available. Qualifying employees are eligible to participate on the first of the month following the date of hire.

Dental Insurance
Delta Dental is our dental insurance provider and coverage is available for employees and their dependents. Qualifying employees are eligible to participate on the first of the month following the date of hire.
Basic Life Insurance/AD&D
Eye Associates provides group term life insurance equal to one times the base annual salary for regular employees (up to certain maximums) hired to work at least 30 hours per week. The plan premiums are paid by Eye Associates. Qualifying employees are eligible to participate in the first of the month following 90 days of employment.
Supplemental Life Insurance
Qualifying employees have the option to purchase additional group life insurance for themselves and their dependents.
Disability Insurance
Eye Associates provides short-term/long-term disability insurance for regular employees hired to work at least 30 hours per week. The plan premiums are paid by Eye Associates. Qualifying employees are eligible to participate in the first of the month following 90 days of employment. The plan is intended to provide partial income replacement for employees who are unable to work due to illness, injury or childbirth.
Long-Term Care Insurance
Regular employees hired to work at least 30 hours per week have the option of purchasing long-term care insurance effective the first of the month following 90 days of employment. The insurance provides a monthly benefit to help offset the costs associated with long-term care provided in a nursing home or in the home.
401(k) Retirement Plan
All Eye Associates employees have the option to contribute a portion of their pay on a pre or post-tax basis into their own accounts within our qualified 401(k) Plan. Investment options include a diverse group of funds.
Flexible Spending Accounts (FSA)
Eye Associates provides regular employees hired to work at least 20 hours per week the option to enroll in either/both a Healthcare and/or a Dependent Care Flexible Spending Account. FSA’s allow employees to make pre-tax contributions that can be used to reimburse themselves for certain medical and/or dependent care expenses, while also lowering their taxable income.
Vision-Related Benefits
Employees are offered a variety of vision-related benefits and discounts, such as eye exams and eyeglasses, as well as discounts on contact lenses, LASIK/PRK and selected oculoplastics procedures/treatments. Qualifying employees are eligible for these benefits on the 91st of employment.
Employee Assistance Program (EAP)
All Eye Associates employees (and their family members) have confidential access to counselors, crisis intervention, and community resource referrals. Eye Associates provides this plan to employees at no cost.
Paid Time Off & Holidays
Eye Associates provides competitive time-off packages to employees. The type of time off plan may differ depending on exempt or non-exempt employment status. In addition, eight paid holidays are designated by the corporation each year.
